An employee or immediate family member of an employee may simply call the EAP to schedule an appointment.

A supervisor may recommend that an employee contact the EAP. If a supervisor refers an employee, he/she should contact the EAP Manager first, then have the employee call to schedule the appointment.

About Confidentiality…

The EAP will uphold the confidentiality of each individual that accesses EAP services, within the limits of state and federal laws concerning confidentiality.
